Coronary Artery Disease

Coronary Artery Disease, also known as CAD, is a common type of heart disease. With CAD, plaque can build up in the arteries over time. This can lead to a heart attack. By following the Care Management Plan from your doctor, you can reduce your risk of serious complications from CAD.

We will help you manage your heart disease by making sure that you have regular visits with the doctor, helping you manage your risk factors, giving you a referral for nutrition counseling if needed, supporting you in smoking cessation, and helping you find the right exercise program for you.

Short Term Goals:

Take medications regularly that have been prescribed by your Doctor.

Maintain a healthy blood pressure

Follow a heart healthy diet: The Mediterranean Diet.

Start an exercise plan, 30 minutes a day, 5 times a week.

Maintain healthy cholesterol levels. Healthy cholesterol levels are

  • Total Cholesterol is less than 200.
  • Triglycerides are below 150
  • HDL is above 40
  • LDL is below 100

Quit smoking.

Talk with your doctor about warning signs of a heart attack and when to call for help.

Long Term Goals:

Maintain healthy blood pressure numbers.

Reduce your weight and/or maintain healthy weight.

A healthy diet and regular exercise are key for reducing risk of a heart attack.
